- Must be 65 years of age or older
- Completed a pre-admission health and financial screening
- Must be without a primary or secondary diagnosis of a developmental disability or a serious and persistent mental illness
- Completed a Determination of Need screening by the Department on Aging
- Negative Tuberculosis test result completed within the last 90 days
